people post a lot about pleroma not respecting suspensions and never seem to post about mastodon not enforcing suspensions, or mastodon not respecting suspensions

I don't even think mastodon tells a suspended instance that they've been suspended. There's nothing for a remote instance to respect. It's just getting posts
Like they won't get posts directly from your instance but if it ends up there via boost or something? They have no idea
I can probably turn on authorized fetch at this point. I think most things support it now. I'll have to check in the morning
If you're doing suspensions as a safety thing for your users and not just to not have to deal with people, you should turn on authorized fetch
